Waivers

Waiver Regarding Medical Emergencies:

I, hereby grant permission to the Director of Monsignor Scanlan High School's Basketball Camp to make necessary medical decisions on behalf of my child, named above, in the event of an emergency. This authorization extends to situations where I cannot be immediately reached.

I understand that the Director will use their best judgment to ensure my child receives prompt and appropriate medical care. In doing so, they may consult with medical professionals and follow their recommendations.

I hereby release and hold harmless Monsignor Scanlan High School, its program staff, Scanlan's Basketball Camp, and any affiliated entities, along with their officers, agents, and employees, from any and all liability or legal action arising from or related to my child's participation in the program, including any medical decisions made in an emergency situation.

This authorization is in effect for the duration of my child's participation in Scanlan's Basketball Camp.

Waiver Regarding Medication:

I, hereby acknowledge and agree to the following terms and conditions regarding the administration of medication to my child, named above, at Scanlan's Basketball Camp, if applicable.

Self-Administration: My child is capable of self-administering any medication prescribed or recommended by a healthcare professional during camp hours.
Medication Waiver: I understand and agree that a completed Medication Waiver Form must be submitted with my child's registration. This form outlines the specific medication(s), dosage, and administration instructions.
Original Containers: All medications, both prescribed and over-the-counter, must be brought to camp in their original, labeled containers.
Labeling: Each medication container must be clearly labeled with my child's name, the medication name, dosage instructions, and any expiration date.
Expired Medications: Scanlan's Basketball Camp reserves the right to refuse expired medications due to safety concerns.
Zip-Lock Bag: For safekeeping, all medications must be placed in a labeled zip-lock bag with my child's name clearly written on it.
Dosage/Schedule Changes: Any changes in medication dosage or administration schedule require written authorization from my child's physician. Updated instructions must be submitted to Scanlan's Basketball Camp promptly.

I understand that Scanlan's Basketball Camp staff cannot administer medication unless explicitly authorized in the completed Medication Waiver Form and with appropriate training.

I, the undersigned, assume full responsibility for ensuring the accuracy of the above information and hold Scanlan's Basketball Camp harmless for any complications arising from the administration of medication to my child during camp hours.

Waiver Regarding Sun Block and Bug Repellent:

I, hereby grant permission for the staff of Scanlan's Basketball Camp, to apply sunblock and/or bug repellent to my child, named above, as needed.

Waiver Regarding Dismissal from the Program:

Campers are expected to follow the rules and regulations set forth by the Scanlan's Basketball Camp program. Failure to adhere to these guidelines may result in dismissal without reimbursement or the possibility to appeal.

For the safety and well-being of all participants, campers are not permitted to leave the school grounds during the program, except in the case of a documented medical emergency requiring immediate attention. All other departures must be authorized by the child's legal parent(s) or guardian(s) and coordinated with camp staff.

Waiver Regarding Photography and Videography:

I, hereby grant permission to Monsignor Scanlan High School to use my child, named above, name, image, likeness, and any recorded audio or video footage of my child, without any compensation or remuneration, for the following ex purposes:

Advertising and promotional materials
Website and social media content
Publications and marketing materials
Educational and informational materials
Any other lawful purpose related to the activities of Monsignor Scanlan High School

I understand and agree that the use of my name, image, and likeness may include, but is not limited to, photographs, videos, audio recordings, and any other media format.

I agree that Monsignor Scanlan High School shall have the right to edit, modify, reproduce, distribute, and publicly display any materials that include my child's name, image, and likeness, without any further approval or consent from me.

I hereby release and discharge Monsignor Scanlan High School, its employees, agents, representatives, and any third parties acting under its authority, from any claims, demands, or liabilities arising out of or in connection with the use of my child's name, image, and likeness as described in this waiver.

I acknowledge that I have read this waiver carefully and understand its contents. I voluntarily and knowingly waive any rights, claims, or interests I may have in relation to the use of my child's name, image, and likeness.

This waiver shall be binding upon me, my heirs, executors, administrators, and assigns.
